Should you wish to distribute a survey to the COSA membership (all members or part thereof) please familiarise yourself with the COSA Survey Policy (pdf).
All requests must be made in writing to cosa@cancer.org.au and include:
- a completed COSA Survey Distribution Form
- relevant supporting documentation
All applications are reviewed by the COSA Survey Review Committee.
Surveys may not be distributed to COSA members by any other means, for example via the COSA Group Noticeboards on this website or social media.
