Should you wish to distribute a survey to COSA members (all members or part thereof) please familiarise yourself with the COSA Survey Policy (pdf).
All requests must be made in writing to [email protected] and include:

- a completed COSA Survey Distribution Application Form (download)
- relevant supporting documentation
Please note: Your application and supporting documentation should be combined into a single PDF file.
All applications are reviewed by members of the COSA Survey Review Panel.
Surveys may not be distributed to COSA members by any other means, for example via the COSA Group Noticeboards on this website or social media.
Please note: Survey requests will not be accepted between 1 December and 31 January each year.

Want to join the
review panel?
The Survey and Research Invitation Review Panel is responsible for review and approval of surveys and research invitations to be distributed to COSA members.
The panel currently consists of 6 members, however, COSA would like to recruit 3-4 more members with diverse interests to broaden the panel, spread the workload, and allow greater flexibility.
Expressions of interest are now being kept open on an ongoing basis, or until we have sufficient members on the panel.
